Home Health Foundation is comprised of three not-for-profit affiliate agencies: Home Health VNA, Merrimack Valley Hospice and HomeCare, Inc. Together, they form a comprehensive continuum of the highest quality home health and hospice care for infants, children, adults, and elders, in both home and community settings. Our highly skilled home care and hospice professionals are renowned for their expertise in a wide range of cutting-edge clinical and supportive services. We serve more than 100 cities and towns of the Merrimack Valley, Northeastern Massachusetts and Southern New Hampshire.
